You receive insurance cards for health, prescription, dental and vision benefits. You can also access your digital ID cards from the My Health Toolkit^®, Caremark and EyeMed^® apps. Only the subscriber’s name will be on the cards, but all covered family members can use them.

Missing one of your insurance cards? Any one of them can be replaced using the contacts listed here. You will need your Benefits Identification Number. If you don’t know your BIN, visit MyBenefits.sc.gov and select “Get My BIN” in the lower right corner.

Health

After your initial enrollment, you will receive two ID cards from BlueCross BlueShield of South Carolina. You can also access your digital ID card from the My Health Toolkit^® mobile app. If you need to order a replacement card, log in to your My Health Toolkit account or call BlueCross at 888.214.6230.

Dental

After your initial enrollment, you will receive a Dental Plus ID card from BlueCross. You can also access your Dental Plus digital ID card from the My Health Toolkit mobile app. If you need to order a replacement card, log in to your My Health Toolkit account or call BlueCross at 888.214.6230.

Prescription

After your initial enrollment, you will receive two ID cards from Caremark. You can also access your digital ID card from the Caremark mobile app. If you need to order a replacement card, log in at www.caremark.com or call Caremark at 833.291.3646.

Vision

After your initial enrollment, you will receive two ID cards from EyeMed. You can also access your digital ID card from the EyeMed mobile app. If you need to order a replacement card, visit www.EyeMed.com or call EyeMed at 877.735.9314.

Caremark is an independent company that contracts directly with PEBA.

EyeMed Vision Care is an independent company that administers vision benefits for PEBA.
